Project Overview:
A University of Michigan–led national poll found that fewer than half of parents (only 43%) have set up a patient portal for their child. They do report benefits like timely access to medical records and simpler communication. However, most parents still rely on fragmented systems, making it difficult to manage children’s academic and healthcare data across schools, clinics, and other providers.
Our client envisioned a care coordination platform for children that would facilitate real-time collaboration among parents, teachers, and healthcare professionals. Folio3 Digital Health delivered a HIPAA- and FERPA-compliant cloud-based care coordination platform that centralized health and educational records, enabling caregivers to communicate and set developmental goals. This child-centered care system improved accessibility across institutions and supported vulnerable populations by ensuring holistic care.
We built a care coordination platform for children that brought together fragmented health and academic records into a unified system. With this secure hub, every aspect of their child’s progress, from medical history to school performance, could be accessed and managed seamlessly.
Real-Time Communication
We integrated easy-to-use communication tools, like chat, discussion boards, and notifications, into our child-centered care platform. These features made it possible for care teams to connect in real time, cutting down delays and improving how decisions are made.
Goal Setting and Tracking
To make the platform child-centered, we introduced goal-setting for development milestones and progress-tracking features. This structured framework provides visibility into a child’s growth over time, ensuring that everyone involved, including parents, educators, and healthcare providers, is aligned in supporting the development.
Secure Infrastructure
The platform uses a HIPAA- and FERPA-compliant infrastructure to meet strict privacy and compliance requirements. End-to-end encryption, secure user authentication, and a compliance-first approach ensured that sensitive child data remained fully protected.
Visible progress in child development: Computer vision imaging enables quicker analysis for timely breast cancer detection.Educational and recreational goal-setting and tracking tools gave caregivers clear insights into outcomes, enabling more effective support for each child.
Up to 40% reduction in emergency visits: Studies show that effective care coordination for children reduces sudden, unexplained decline in health and improves care integration.
50% less time spent coordinating care: Families report significantly reduced time and stress managing appointments and records when care coordination is in place.
Faster caregiver collaboration: Real-time chat, discussion boards, and notifications improved communication among parents, teachers, and healthcare professionals, leading to quicker, more informed decisions.
