How Does Epic Care Everywhere Work?
Epic Care Everywhere works by sending a standards-based query to another organization, matching the patient, and returning a clinical document that the local Epic system parses into the chart. It is not a live view into another hospital’s database.
It is a request-and-response exchange of documents, governed by national standards and trust frameworks. Understanding that distinction is the difference between expecting a magic window and understanding what you will actually get back.
Exchange Standards Used
Epic Care Everywhere uses established interoperability standards.
- For query-based exchange it relies on the XCA and XDS.b integration profiles from Integrating the Healthcare Enterprise (IHE).
- Planned transitions of care uses Direct Project standards.
The documents themselves follow HL7‘s Clinical Document Architecture, which is why the payload comes back as a C-CDA rather than discrete, field-level data.

Accessing Care Everywhere in Epic
You access Care Everywhere in Epic from inside an open patient encounter, through the Care Everywhere activity, then the Request Outside Records action.
It is not a separate application you launch on its own. It lives in the chart, attached to the encounter, which is why access depends on your role and the encounter type as much as on the software itself.
- Open an encounter of a type your organization has authorized for Care Everywhere querying, such as an outpatient appointment, an admission, or a notes-only encounter.
- Go to the Care Everywhere activity, shown as a tab or found under the More button, and select Request Outside Records.
- If the outside organization is not already linked to the patient, choose Query New Organization, type the organization name, and enter your Epic password if prompted.
- Verify the patient against the highlighted demographics, then confirm with Yes, Correct Patient, or reject a mismatch.
- Once linked, review the outside summaries, office visits, and test results, and use Request Updates to refresh the query later.
Finding Care Everywhere ID in Epic System

But When Do You Actually Need the ID?
Usually, the query matches the patient automatically on demographics. The Care Everywhere ID becomes useful when the automatic match fails. In the SF DPH process, when no match is found, staff are told to call the other organization’s health information management department, request the patient’s Care Everywhere ID, and follow the Epic prompts.
Opting Out of Epic Care Everywhere
Patients are enrolled in Epic Care Everywhere automatically, and opting out requires a written request to the organization’s health information management department.
There is no sign-up step to undo, because sharing for treatment is permitted by default. To stop it, a patient has to actively opt out, and the mechanics are worth understanding before anyone assumes opt-out means their data disappears.
- Tell a registration or patient access staff member you want to complete a Care Everywhere opt-out request, or download the organization’s opt-out form.
- Submit the signed form to the health information management or medical records department.
- Allow processing time, which one large system lists as up to five days and others as up to ten business days.
- Understand you can reverse the decision and opt back in at any time with a new request.
What Opting Out Does Not Do?
Opting out stops future electronic sharing but does not pull back records already transmitted to another institution. It does not block providers from getting your information by other lawful means, such as phone, mail, or a direct medical-records request.
State and federal law still permit treatment-related disclosure. Opting out of Epic Care Everywhere narrows one electronic channel. It does not make a record private in every sense.
The Epic Care Everywhere Blacklist Function
The Epic Care Everywhere blacklist function is the organization-side configuration that lets a health system block specific trading partners from querying its records, even though the network is open by default.
It is the counterweight to on-by-default exchange. The network assumes everyone can talk to everyone, and the blacklist feature is how an individual organization carves out exceptions.

1. Patient Matching Failure
Every query depends on the two organizations agreeing that this is the same human being, matched on demographics. Names change, dates of birth get fat-fingered, and a common name in a dense region produces ambiguity.
When the match fails, Care Everywhere does not throw a loud error. It returns nothing, and unless the clinician knows to chase a Care Everywhere ID through the other site’s HIM team, the outside record simply never appears.
2. Documented Data is not Discrete Data
Because the exchange rides on CDA and returns C-CDA summaries and sometimes non-discrete PDFs, the data that arrives is not always something your system can file at the field level.
A clinician may get a readable summary that still has to be reconciled by hand, and a medication or problem buried in a PDF does not flow into structured decision support. The Epic Care Everywhere interoperability story is strong on reach and weaker on the granularity of what actually lands in the chart.
3. Non-Epic Exchange Lags Epic-to-Epic
Epic-to-Epic exchange has many document types available. Reaching non-Epic systems through national frameworks works, but the depth often drops, because less mature implementers on the other end may expose only basic core clinical data.
So a query to another Epic site might return encounter summaries, specialty documents, and chart notes, while the same query to a non-Epic organization returns a thin patient summary. The network connects them. It does not equalize what each side is willing or able to share.
